Licensed Certified Social Workers are integrated in our primary clinics to offer assistance for patients struggling to cope with mental health, substance abuse and chronic diseases. A Behavioral Health Community worker can assist high risk patients by facilitating the transition from hospital to home, establishing care with a primary medical home, transportation assistance and home visits.
