Licensed Certified Social Workers provide assistance for patients struggling to cope with mental health, substance abuse and chronic diseases. A Behavioral Health Community worker can assist high risk patients by facilitating the transition from hospital to home, establishing care with a primary medical home, transportation assistance and home visits.
"Our providers are very personable and always try to make the patients feel welcome. Our nurses have all the answers in the world, and if they don't, they can surely find them. The social workers at ECHO have a patience that I have never experienced before. All the employees are close and friendly, and we are definitely a 'work family' and not just co-workers."
