HOMELESS HEALTH OUTREACH
The Community Case Management Team (CCMT), is made up by a social worker and an RN. They provide medical and psychosocial screenings at soup kitchens, shelters and food pantries throughout Vanderburgh County. Patients in need of in-depth medical services are referred to one of our four ECHO clinics and collaborate with other community agencies to meet the needs of this vulnerable population.
"It has been said that, 'If you don’t have your health you don’t have anything.' People often believe that being homeless is just not knowing where you are going to lay your head from one night to another. It is often times not knowing where your next meal is coming from, or even worse yet, not knowing what will happen if you become ill. ECHO takes care of these health issues that most people never even consider. Most people in our society rarely stop and consider how much their health means to them. To someone that is homeless, it may literally be all they have. That’s where ECHO comes in."
