The Community Case Management Team (CCMT) is made up of a Case Manager, an RN, and a Multicultural Liaison. The CCMT provides medical and psychosocial screenings at soup kitchens, shelters, and food pantries throughout Vanderburgh County. Patients in need of further medical services are referred to one of our clinics. We also collaborate with various local social service agencies to help meet the needs of this vulnerable population.

At ECHO we are not solely a healthcare center, but we offer a wide variety of other on-site services. Each of our locations has case managers and health therapists in order to better serve these populations, or any population in need of services or in crisis.
