Paid Time Off (PTO)

In accordance with ECHO policy, unused PTO will be paid out on your final paycheck if you have been employed at least two years continuously and have given sufficient notice. Your final paycheck, including all earned wages through your separation date, will be issued in accordance with standard payroll procedures.

Medical, Dental, and Vision Insurance

All will term at end of month of last day worked.

Life Insurance

Your $65,000 group life insurance benefit, provided by ECHO, will terminate at the end of the month of the last day worked. You may be eligible to convert this coverage to an individual policy. Please click HERE to learn more. Any election must be completed within 31 days of your termination date.

Disability Insurance

Long-term disability coverage (company-paid) will end at the end of the month of the last day worked. These benefits are not eligible for conversion.

403(b) Retirement Plan

Paylocity Access

You will retain access to Paylocity for 12 months following your separation date for purposes of retrieving electronic W-2 forms and payroll information. Paylocity may be accessed at Paylocity.com

Continuity of Care

Former employees who receive primary medical or dental care through ECHO may continue to utilize ECHO as their medical home; however, ECHO reserves the right to require reassignment to a clinic location other than the site where the individual was employed.
